Peer Specialist at Empowerment Apts. at Greyhound Park

The Empowerment Program is a Denver-based nonprofit that removes barriers to health, housing, education, and employment so individuals can live a healthier life. Since 1986, we have worked alongside our participants, primarily cis and transgender women who have lived experiences with the criminal legal system or homelessness, to connect them with the services they need to achieve their goals. Since our founding, we’ve redefined “Empowerment” as a form of collective action.

General Purpose:

Overall support for the various programs, grants, and organization and front desk coverage.

​Job Functions
  • Assist clients in articulating personal goals for recovery using one-to one and group sessions. During these sessions the PS will support clients in identifying and creating goals and developing recovery plans with the skills, strengths, supports and resources to aid them in achieving those goals
  • Assist clients in working with their case manager or treatment team in determining the steps he/she needs to take to achieve these goals and self-directed recovery.
  • Assist clients in setting up and sustaining self-help (mutual support) groups, as well as means of locating and joining existing groups.
  • Utilize tools such as the Wellness Recovery Action Plan (WRAP) to assist clients in creating their own individual wellness and recovery plans.
  • As much as possible, the PS will share their own recovery story and as the facilitator of these sessions, will demonstrate how they have directed their own recovery.
  • Assist clients in building social skills in the community that will enhance job acquisition and tenure.
  • Utilizing their recovery experience, the PS will:
  • Teach and role model the value of every individual’s recovery experience.
  • Assist the client in obtaining decent and affordable housing of his/her choice in the most integrated, independent, and least intrusive or restrictive environment by taking them out to view housing, either driving them or riding with them on public transportation.
  • The PS models effective coping techniques and self-help strategies.
  • Serve as a recovery agent by providing and advocating for any effective recovery-based services that will aid the client in daily living.
  • Assist in obtaining services that suit that individual’s recovery needs by providing names of staff, community resources and groups that may be useful. Inform clients about community and natural supports and how to use these in the recovery process. Community resources may include but not limited to: social security office, Department of Family and Children services, local YMCA, Library, restaurants, clients’ service organizations, apartment complexes and other types of housing, etc.
  • Assist clients in developing empowerment skills and combating stigma through self-advocacy. This will be accomplished through regular meetings, individual or group sessions. Using role playing/modeling techniques the PS provides opportunities for others to show/demonstrate how they have handled similar problems, how to present themselves in certain situations, or how to handle problems that may arise in interactions with others.
  • With assistance from the Program Coordinator/Team Leader, the Peer Specialist will work with the clients and other treatment team staff to develop a treatment/recovery plan based on each client’s identified goals. Treatment/Recovery Plans will be reviewed and signed by the Coordinator/Team Leader and other participating treatment team staff.
  • The PS will document the following on the client’s treatment/recovery plan:
  • a. identified person-centered strengths, needs, abilities, and recovery goals
  • b. interventions to assist the client with reaching their goals for recovery

Knowledge Required by the Position
  • Knowledge of the Recovery process and the ability to facilitate recovery using established standardized mental health processes.
  • Knowledge and skill to teach and engage in basic problem-solving strategies to support individual clients in self-directed recovery.
  • Knowledge of the signs and symptoms of mental illness (i.e. auditory and visual hallucinations, aggressive talk and behavior, thoughts of self-harm or harm towards others, isolation) and the ability to assist the client to address symptoms using strategies such as positive self-talk.
  • Knowledge and skill sufficient to use community resources necessary for independent living and ability to teach those skills to other individuals with severe mental illness. Community resources may include but are not limited to: social security office, Department of Family and Children services, local YMCA, Library, restaurants, clients’ service organizations, housing providers, etc. The PS may accompany clients to community resources to assist them in accessing these resources.
  • Knowledge of how to establish and sustain self-help (mutual support) and educational groups by soliciting input from mental health consumers on their strengths and interests.
  • A valid driver’s license is required as some driving and/or transportation may be required to take clients to medical appointments, job sites, social activities, and other community resources.
  • Front desk coverage at Greyhound on a regular shift schedule
  • Many other various daily tasks

Housing Care Manager

Empowerment Program provides housing and services to at-risk communities, such as people with disabilities, mental illness, or substance abuse, working from a trauma informed lens. This position is part of a team that coordinates housing services provided to participants of the Empowerment Program. This is a Housing First program for women who were formerly homes. The participants present with varying degrees of intensity. The goal of the program is self-actualization of residents, providing framework for residents to work towards self-sufficiency to the greatest possible extent, recognizing the strengths and limitations of each resident.

 Primary Responsibilities: 
  • Become familiar with CHFA, Housing First, and Fair Housing Rules.
  • Providing services and support to participants, often in their homes.
  • Pursuing and engaging with reluctant participants, in a trauma informed manner.
  • Maintaining accurate, timely and detailed documentation on all services provided to residents.
  • Maintain resident confidentiality and comply with code of ethics. 
  • Educate residents about existing or new housing resources and services.

Part-Time Residential Staff – On Call Position


Job Overview
The Empowerment Program provides health, housing, and employment and education services to women and transwomen who are involved in the criminal justice system, have been recently released from jail or prison, are homeless, living with HIV/AIDS, mental illness, and drug addiction.
Our house is a confidentially located congregate living house for 18-22 women Denver.  This is an on-call, part-time position providing coverage as needed on weekdays, weekends, and overnight coverage.
 
Essential Job Functions:
  • Maintain safety and security by monitoring all general access areas.
  • Enforce the house rules as outlined in the handbook.
  • Arm the security system each night at curfew and turn it off in the morning.
  • Notify the Housing Manager of maintenance or safety/security concerns.
  • Intervene in crises, respond to emergencies, and initiate action as required, including contact with emergency response systems.
  • Email significant events to the housing team and maintain daily logbook.
  • Create and monitor daily chore assignments.
Position Qualifications:
  • Lived experience is helpful.
  • One year of direct residential experience, preferred. 
  • Commitment to helping homeless and formerly homeless women to regain and maintain independence.
  • Those with a history of homelessness or involvement in the criminal justice system are encouraged to apply.

Medicaid Billing Specialist

Job Overview
The Medicaid Billing Specialist will work as part of a small accounting team for the Empowerment Program which is a non-profit organization providing behavioral health treatment and services – this position will include billing Medicaid for the agency.
Job Responsibilities:
  • Collects Medicaid reimbursements by gathering, coding, and transmitting patient care information.
  • Keeps up to date on information required for remittance of patient care by Medicaid and communicates this information to fellow billing staff.
  • Reviews patient hospital records to check for proper Medicaid coding and completeness and makes corrections accordingly.
  • Analyzes patient billing records for completeness and accuracy and obtains additional information and clarification as necessary.
  • Resolves billing discrepancies by conducting further research and correcting errors.

Medical Care Manager

Job Description
This position works fulltime and directly with women who are living with HIV/AIDS.  Comprehensive, individualized case management services are provided to women and their families in the home or community.  Care management utilizes a participant-centered process that links clients to housing, health care, mental health, substance abuse, and other support services to produce more positive and sustained outcomes. The care manager will establish a service plan, develop continuity of care, and coordinate the services required to implement the plan.
 
The roles include:
  • Providing intensive participant-centered care management services for women living with HIV with the goal of improving health outcomes; viral suppression and/or increased CD4 counts
  • Completing individual program intakes, including Initial assessment of needs and development of a comprehensive individualized service plan
  • Ongoing assessment of participant strengths and needs, including monitoring of the individualized service plan and re-evaluation every 6 months. 
  • Linkage to care and coordinating care as needed 
  • Assessing health literacy, adherence to appointments and medications, and medical self-management skills
  • Advocating for participant’s rights in healthcare decisions and addressing barriers to care and treatment
  • Identifying, prioritizing, and addressing non-medical needs and linking participants to appropriate community resources
  • Assessment of medical transportation needs; linkage to community resources and distribution of medical transportation services (i.e. bus tickets, cab vouchers)
  • Completing necessary documentation that is in compliance with grant the administrator; this includes but is not limited to data entry, progress notes, service plans, assessments, and eligibility documentation
  • Maintaining participant confidentiality
  • Attending case conferences with medical providers to discuss participant strengths, needs, barriers to care, and collaboration to engage and/or maintain participants in care.   
  • Attending community meetings with other organizations that serve PLWHA to improve service delivery, collaborate services, discuss standards of care, and to support community.
  • Attending educational training related to HIV and PLWA
  • Assisting in organizing and facilitating psycho-social events for women living with HIV to facilitate education, access to health and other benefits, improve coping skills, reduction of isolation, and to increase self-determination and advocacy.

Housing Program Manager

Position Overview
The Housing Program Manager is responsible for overseeing effective operating procedures throughout the main Empowerment site and care management of participants living in Empowerment Housing. Their duties include working with the HR department and Executive Director to implement and improve building and housing operating procedures and management of staff and a housing location working with The Housing Director and Deputy Director. This is a full-time, regular, exempt management position that at times requires work weeks more than 40 hours and may require evening and weekend work on occasion.
 
Responsibilities for Housing Operations and Care Manager
  • Defining, implementing, and revising operational policies and guidelines for Empowerment at 1600 York (your main office), and assist Empowerment employees with maintaining a clean and healthy environment at 1600 York.
  • Developing and executing new building growth directives
  • Ensure operational activities remain on time and within a defined budget.
  • Liaison between cleaning company and 1600 York
  • Assist Deputy Housing Manager with Yardi program and compliance to include COC billing and Rent roll input
  • Support Deputy Housing Director with day-to-day management of residential programs including rent collection, lease completion, create and maintain resident files as required by funding sources.
  • Sign leases and contractual agreements COC and with residents with section 8 vouchers and market residents.
  • Support Care Managers with regular check-ins as needed.
  • Deal with all issues (drug dealers, traffickers, dv) and issues with residents in Mariposa 24/7
  • Coordinate all resident moves with outside movers
  • Establishes and maintains credible, professional relationships with clients, internal business lines, and external vendors.
  • Communicates inventory needs to the Deputy Housing Director by checking stock to determine inventory level; anticipating needed building supplies; evaluating new office products; and verifying receipt of supplies and proper distribution of products.
  • Become familiar with CHFA and Fair Housing Rules
  • Be on-site at the Mariposa house and have in person contact with residents a minimum of twice per week. 
  • Maintain resident confidentiality and comply with code of ethics.
  • Educate residents about existing or new housing resources and services. 
  • Directly supervise Mariposa house staff and communicate effectively with Care managers who have participants within Mariposa House and the Housing Director
  • Report directly to Deputy Housing Director.
  • Maintain mutually respectful, collaborative relationships with staff and residents, acting as a positive team member.

Substance Use Counselor

Job Description: CDPHE RW Part B - Women's Health Project (WHP). The WHP Substance Use Therapist position works full-time with women who are living with HIV/AIDS. Comprehensive, individualized substance abuse services are provided to these women. The WHP Substance Use Counselor works with the participant’s care managers who utilize a participant-centered process that links clients to housing, health care, mental health, substance abuse, and other support services to produce more positive and sustained outcomes. The WHP Substance Use Counselor will establish a service plan, develop continuity of care, and encourage the participant to work with their care manager to coordinate the services required to implement the plan. This position also works directly with women who have been referred to Part B transitional housing at the Mariposa House.  The WHP Substance Use Counselor also provides on-going weekly evidenced based substance use groups, connects women who are seeking sobriety with opportunities for recovery-based events such as attending exercise at The Phoenix- a free gym for those who are abstaining from the use of alcohol and other substances.

The Substance Use position is responsible for:
  • Providing substance use individual sessions for women living with HIV/AIDS
  • Identifying stages of change regarding substance use by conducting assessments such as the SBIRT and ASSIST.

Peer Support Specialist - Housing Opportunities for People Living with HIV/AIDS (HOPWA)

Job Description:
The HOPWA Peer Support Specialist works directly with women living with HIV who are either homeless or housed to maintain and/or acquire housing. The Peer Support Specialist would be available to assist in any life skills support services and education that will benefit women to live well independently and with their families, to access needed supports to maintain housing, and to help women access all needed eligibility requirements to access housing throughout the community. The Peer Support Specialist will work with current medical care managers, Empowerment housing staff, community landlords and resources/supports to ensure participant needs are being met.

Job Functions
  • Understanding HIV basics
  • Understanding and implementing the HOPWA Scope of Work and completing required documentation

Mariposa House Residential Night Staff

This position provides night staffing for 12 hours on Saturday, and possible weekend days or nights on call coverage for a congregate living house for up to 18 cis and transgender women. General duties provide safety and security for house and the women we serve. Some de-escalation of difficult situations may be required. Must be able to navigate stairs.

Position Duties and Qualifications:
  • Knowledge if trauma informed practices and harm reduction models preferred.
  • Reliable transportation as we are a 24/7 facility, and we are open during bad weather.
  • Maintain safety and security by monitoring all general access areas.
  • If it is suspected that a resident may be under the influence of drugs, call Detox or Paramedics to determine the need for medical clearance, transportation to Detox, or possibly a mental health hold.
  • Notify the appropriate staff of maintenance and safety/security immediately after they are identified. If it is not urgent, emails are appropriate.
  • Report emergencies to proper authorities, and document critical incidents on the Critical Incident Form.
  • Meet with Empowerment care managers, as needed.
  • Intervene in crises, respond to emergencies, and initiate action as required, including contact with emergency response systems.
  • Document significant events involving residents (i.e. seizures, arguments, etc.) and building operations activities in a daily email; read emails daily and send a summary of resident attendance on the Nightly Shift Report at the end of each shift.
  • Do not allow anyone who is not staff, parole, or residents into the house.
  • Contact DPD if there are any intruders.
  • Arm the security system each night at curfew and turn it off in the morning. Check status if an alarm goes off. If there is reason to believe there is an intruder, call 911.
  • Bilingual Spanish is preferred but not required.

Residential Property Manager

This position is a “live-in” manager at the Lowry Columbine Apartments. The manager is responsible for on-site management of 17 apartments, which houses formerly homeless residents, some with disabilities and mental health concerns and potential substance use disorders. We are a Housing First program.  This is an unsalaried position which provides an onsite three-bedroom townhome/apartment, and utilities. The Resident Manager (RM) facilitates the efficient operation of Empowerment Housing facilities and works directly with the Empowerment Housing Director (EHD) and Empowerment Housing Manger (EHM) to assure the safety and security of residents. 
 
Primary Responsibilities: 
  • Maintain safety and security by monitoring all general access areas and enforcing the Community Handbook. 
  • Treating residents with respect and having an expert level understanding of trauma informed care is required.
  • Respond to resident complaints, while maintaining professional boundaries and courteous interactions. 
  • Mediate conflicts or disputes between tenants and inform the EHD who then informs the resident’s Case Manager. 
  • Secure grounds regularly including during the night. Check vacant apartments to be sure they are secure. 
  • Maintain and review security cameras regularly. Report concerns to EHD. 
  • Work with EHD and STAR/DPD to remove intruders. Mediate conflicts or disputes between tenants and inform the EHD who then informs the resident’s Case Manager. 
  • Notify the EHD and EHM of maintenance and safety/security immediately after they are identified and/or brought to the Resident Manager’s attention. 
  • Deep clean in common areas, including vacuuming and steam cleaning clubhouse and stairways, sweeping, mopping, and cleaning laundry area and back stairs, and keeping the outdoors property clean, and free of trash and debris. Shovel snow under 2-inches. Clean up salt after area is dry.  
  • Report emergencies to proper authorities, and to the EHD/EHM. Submit maintenance request and incidents, in writing, to EHM.  
  • Work cooperatively with clinical and maintenance staff at the Empowerment Program or other mental health and/or drug treatment and other approved providers to provide services to residents. Facilitate onsite activities and food distribution. 
  • Write significant events involving residents and building operations activities in a weekly email to the EHD. Submit Critical Incident reports for serious events. 
  • Assist with room turnovers, facilities inspections, and maintenance contractors. 
  • Assist in an annual inspection of all apartments which do not turn over in a calendar year to insure the proper maintenance requests of each unit. 
  • Be available to assist the EHD/EHM in showing apartments to prospective residents. 
  • Collect keys from residents when they vacate, maintain key control of the apartments. 
  • Address smoke alarm/fire alarm issues with resident and fire department.   
  • Be available to assist residents who are locked out of apartments. 
  • Maintain and protect confidentiality of the tenants. HIPPA compliance is required. There should be no discussion about residents with non-staff members or other residents.  
  • Assist with other property management functions, including light maintenance, as assigned.  
  • Distribute information and correspondence to residents. 
  • Provide general supervision of the community in the assigned area of responsibility by being visible and available and by establishing and maintaining good rapport with the residents. 
  • Be available during Empowerment Program work hours for meetings and consultations.  
  • Perform other duties assigned by the EHD/EHM.  
 
Terms of Employment: 
1. Remuneration: The Resident Manager will receive the equivalent of an unfurnished, 
Three bedroom-apartment rent-free and utilities paid while employed as the Resident Manager. The RM agrees to maintain telephone service and to have the Voice Mail service available.  
2. It is the RM’s responsibility to notify and seek guidance from the EHD/Housing if at any time, they are uncertain of their responsibilities, or is unable to fulfill these responsibilities. 
3. The EHD must approve other job commitments that are in addition to the Resident Manager responsibilities, to assure employment obligations can be met. 
4. The RM is accountable to the EHD/EHM and the Executive Director.   
 
Qualifications: 
  • Preferred: At least one-year experience in human services, preferably working with homeless women living with mental illness, the economically disadvantaged or chemically dependent.

HIV Prevention Specialist (PrEP Navigator/ HIV Tester)

State Opioid Response (SOR) Peer Support Specialist

The SOR Peer Support Specialist (PSS) will refer individuals with Opioid Use Disorder (OUD) to Medication Assisted Treatment (MAT) and other necessary treatment services.  PSS will work with participants with whom they share lived experience.  Some of the PSS efforts will focus on participants in the criminal justice system.  PSS assists participants in developing coping strategies, help individuals engage in treatment, and provide referrals to community supports. 
 
Job purpose
The Office of Behavioral Health (OBH) received federal funding from the Substance Abuse and Mental Health Services Administration’s (SAMHSA) State Opioid Response (SOR) grant to help individuals in Colorado receive opioid use disorder treatment.  Primary activities and focus is to connect people to MAT and to distribute more naloxone kits.
 
Duties and responsibilities
  • Recruit and provide referral to MAT for at least 5 individuals each month.
  • Recommend appropriate resources and agenices for individuals.
  • Collaborate with case managers to best serve new individuals
  • Must document accurately and in a timely manner participant progress through progress notes, service plans and follow-up contacts according to agency policy and procedure. 
  • Provide referral, linkage, and advocacy for individuals assisting them in obtaining needed benefits and resources including, but not limited to, housing, social security, employment, medical, recreation, social and family supports, nutrition and wellness information and medication.
  • Assist other staff in identifying program and service environments conducive to recovery. Assist staff in retaining participant engagement
  • Help individuals to recognize and utilize their own strengths, skills, talents and abilities to increase the participant ’s independence and self-sufficiency in the community.

Apartment Maintenance Technician

Job Description: 
This position oversees the day-to-day running of five (5) apartment buildings.  The Apartment Maintenance Technician’s responsibilities include cleaning communal areas, and performing ad hoc painting, plumbing, electrical and carpentry duties.  You should also resolve tenants’ maintenance-related queries and complaints as they arise.   Must be able to organize Work Orders and be able to complete in a reasonable time.

HIV Prevention Program Manager

The Empowerment Program is seeking a full-time Program Manager to work with other providers to plan and implement HIV/HepC/STI prevention activities focusing on both HIV positive and negative individuals living with substance use disorder and/or co-occurring disorders.
 
Duties and responsibilities 
  • Supervise program staff
  • Oversee day to day activities and services of the project
  • Establish, coordinate, and facilitate all meetings and work groups
  • Develop and implement community outreach activities to reach individuals in the target population
  • Prepare reports and presentations for review by the contract monitor at CDPHE and other staff, as seen fit
  • Attend meetings with CHAPP as well as the Colorado HIV/AIDS Prevention and Care Alliance
  • Provide oversight on data entry into the Redcap data system
  • Work with the community at large to develop and maintain a community relation

Qualifications

Non-Medical Care Manager


​Job Description
: Non-medical Care Manager will provide trauma-informed, individualized, intensive care management and supportive services to individuals who are living with HIV/AIDS.  The Medical Care Manager will utilize a participant-centered process to help link participants to housing, health care, mental health, substance use disorder, and other support services to produce more healthy and sustained outcomes. The Medical Care Manager will establish a service plan with each participant, develop continuity of care, and coordinate the services required to implement the plan.

Medical Care Manager is responsible for:
  • Providing intensive participant-centered care management services for individuals living with HIV with the goal of improving health outcomes
  • Completing individual program intakes, including initial assessment of needs and development of a comprehensive individualized service plan
  • Ongoing assessment of participant strengths and needs, including monitoring of the individualized service plan and re-evaluation every 6 months. 
  • Linkage to care and coordinating care as needed 
  • Assessing health literacy, adherence to appointments and medications, and medical self-management skills
  • Addressing barriers to care and treatment, including advocating for participant’s rights in healthcare decisions
  • Identifying, prioritizing, and addressing non-medical needs and linking participants to appropriate community resources
  • Assessment of medical transportation needs; linkage to community resources and distribution of medical transportation services (i.e. bus tickets, cab vouchers)
  • Completing all required documentation, including but not limited to data entry, progress notes, service plans, assessments, and eligibility documentation

Recovery Support Services (RSS) Peer Support Specialist

The RSS Peer Support Specialist (PSS) will provide wraparound recovery support services to individuals facing significant barriers to self-sufficiency, including substance use, mental illness, incarceration, homelessness, poverty, and involvement with the criminal legal system. PSS will work alongside participants and care managers to provide non-clinical support across all programming, which includes behavioral and physical health services, emergency and long-term housing support, skills training, and employment assistance.
Specific goals of the Peer Support Specialist includes serving an additional 100 clients per year and supporting participants to achieve improved outcomes in the arenas of income, health, housing, purpose, and community integration, in alignment with the SAMHSA definition of recovery.
 
Job purpose
PSS will be equipped to oversee and streamline all peer support-related initiatives currently in place at Empowerment, as well as to begin new projects within existing programs as opportunities and needs are presented by the community served.
 
Duties and responsibilities
  • Manage case load of around 20-30 participants enrolled in services.
  • Recommend appropriate courses and activities for new participants
  • Collaborate with case managers to best serve new participants
  • Must document accurately and in a timely manner participant progress through progress notes, service plans and follow-up contacts according to agency policy and procedure. 
  • Provide referral, linkage, and advocacy for participants assisting them in obtaining needed benefits and resources including, but not limited to, housing, social security, employment, medical, recreation, social and family supports, nutrition and wellness information and medication.
  • Assist other staff in identifying program and service environments conducive to recovery. Assist staff in retaining participant engagement
  • Help participants to recognize and utilize their own strengths, skills, talents and abilities to increase the participant ’s independence and self-sufficiency in the community.
  • Meet with new participants to conduct intakes.
  • Attend weekly staff meetings.
  • Assist staff with occasional social media postings.
  
Performs other duties as required and/or assigned by supervisor and/or executive staff. 
 
Qualifications
This position will be held by a person who has sufficiently progressed in their own recovery from substance abuse and/or mental health disorders and is willing to self-identify as a peer and work to help others with similar challenges. Minimum 1 year sobriety (current MAT acceptable.)

Behavioral Health Therapist for the Women’s Health Project (WHP)
