1. To enroll in the Empowerment Program, fill out a Pre-Intake Form online. 
2. After you have filled out your Pre-Intake Form, an Intake Specialist will contact you within 2 business days.
3. At that time, the Intake Specialist may help you get enrolled in the program by setting up your secure Patient Portal or setting up an in-person intake.  *If we do not provide the services you are looking for, we can help connect you with resources that better fit your needs. 
4. Once you are enrolled in the program and all the forms are completed and signed, you will be matched with a Care Manager who can provide you with internal and external resources and referrals, and together you will create a service plan.

You do not need to be enrolled to GET TESTED!
